• Using Point-of-Care Ultrasound to Characterize Acute Inguinal Swelling of Young Children in the Pediatric Emergency Department

29 days ago - By Pediatric Emergency Care

Acute inguinal swelling in young children is frequently assumed to be an inguinal hernia, often prompting a bedside reduction attempt. We report 3 cases of inguinal swelling where the use of point-of-care ultrasound changed the patients' management by identifying an alternate diagnosis, thus avoiding unnecessary and painful procedures as well as their associated sedation risks.

  • C-Reactive Protein Concentration Can Help to Identify Bacteremia in Children Visiting the Emergency Department: A Single Medical Center Experience

C-Reactive Protein Concentration Can Help to Identify Bacteremia in Children Visiting the Emergency Department: A Single Medical Center Experience

29 days ago - By Pediatric Emergency Care

    Background
For febrile children who are evaluated in a pediatric emergency department , blood culture can be considered the laboratory criterion standard to detect bacteremia. However, high rates of negative, false-positive, or contaminated blood cultures in children often result in this testing being noncontributory. This study determined the factors associated with true-positive blood cultures in children.
    Methods
This retrospective study was conducted at a tertiary medical center's PED. The blood culture use reports were prepared by an infectious disease specialist and were classified...

  • Poor Agreement but Good Predictive Value Between Automated and Manual QTc Intervals in Pediatric Emergency Department Electrocardiograms

Poor Agreement but Good Predictive Value Between Automated and Manual QTc Intervals in Pediatric Emergency Department Electrocardiograms

29 days ago - By Pediatric Emergency Care

    Objectives
QTc interval is significant because prolongation may lead to ventricular dysrhythmia. Computerized electrocardiogram machines typically measure QT interval length and, using an algorithm assessment of multiple leads, calculate a QTc value. Manual measurement of the QT interval used to calculate the QTc value is more time-consuming but potentially more accurate. In this study, we compare the automated QTc calculation with the QTc value calculated using manual QT measurements.
    Methods
We prospectively obtained 350 resting 12-lead electrocardiograms in children aged 2 to 14 years...

  • Patient-Reported Pain Outcomes for Children Attending an Emergency Department With Limb Injury

Patient-Reported Pain Outcomes for Children Attending an Emergency Department With Limb Injury

29 days ago - By Pediatric Emergency Care

    Objectives
The aim of this study was to describe patient-reported pain outcomes at various stages of an emergency department visit for pediatric limb injury.
    Methods
This prospective cohort consisted of 905 patients aged 4 to 17 years with acute limb injury and a minimum initial pain score of 4/10. Patients reported pain scores and treatments offered and received at each stage of their ED visit. Multiple logistic regression was used to identify predictors for severe pain on initial assessment and moderate or severe pain at ED discharge.
    Results
The initial median pain score was 6/10 and...

  • Novel Approach to Emergency Departments' Pediatric Readiness Across a Health System

Novel Approach to Emergency Departments' Pediatric Readiness Across a Health System

29 days ago - By Pediatric Emergency Care

    Study Objective
A gap analysis of emergency departments' pediatric readiness across a health system was performed after the appointment of a service line health system pediatric emergency medicine quality director.
    Methods
A 55-question survey was completed by each eligible ED to generate a weighted pediatric readiness score. The survey included questions regarding volume, ED configuration, presence of a pediatric emergency care coordinator , quality initiatives, policies and procedures, and equipment. Surveys were completed from June 1 to November 12, 2016.
    Analysis of variance was used...
